Eyelid reduction, also known as blepharoplasty, is an operation to remove the excess skin and/or fat from around your eyes so that you have a more alert and youthful appearance. Eyelid reduction can be carried out on the upper, lower or both eyelids.

The initial consultation is very important. Your surgeon will need a complete medical history. This will include information about any medical conditions, drug allergies, previous operations and medication you are taking, if any. She will examine your eyes.

Preparation for surgery
Smokers will be asked to stop smoking 2 weeks before surgery. Aspirin and some anti-inflammatory drugs can cause increased bleeding, so you should avoid taking these medications for 2 weeks before surgery.

Your Eyelid Surgery Operation

  • Both upper and lower eyelid surgery can be carried out under local anaesthesia or under general anaesthesia in a hospital. It takes 1-2 hours to perform.
  • In upper eyelid surgery, the incision is well hidden within the natural fold of the upper eyelid crease and it is through this incision that excess skin and fat can be removed. Because the incision follows the natural contour of the upper eyelid, it will be well camouflaged when it is healed.
  • In the case of lower eyelid surgery, your surgeon will discuss two different approaches: one which utilizes an external approach and the other using an incision from inside the lower lid margin. Your age, overlying skin condition, degree of lid laxity and the extent of the fatty deposits will determine which approach is recommended.
  • The external approach incision is usually well hidden below the lower lashes. Through this incision, excess skin, muscle and fat are removed. Fat may also be redistributed or sutured in a new location to help eliminate or reduce the puffiness or bulges. Other adjustments such as lid tightening (canthopexy) may be performed to help correct special problems such as muscle laxity.
  • In cases where the lower eyelids contain bulging fat but with no skin excess, then the fat can be removed by an incision entirely within the inside of the eyelid. This is called a transconjunctival blepharoplasty. Here, there are no external scars and the wound is stitched with dissolving sutures.

After you Eyelid Surgery – Blepharoplasty

  • After your surgery, you may have a feeling of dryness or irritation in the eyes which may require medical treatment. There is also a chance of a temporary reduction in eyelid sensation or impaired eyelid function.
  • Brusing and swelling can last for several weeks following surgery.
  • Eye ointment is usually prescribed for use after operation. The eyes may be cleaned with warm water after 48 hours.
  • It is prudent to avoid vigorous exercise for 1 week after surgery and not to bend down for the first 3 days following surgery.
  • Sitting up at night as much as possible reduces swelling that may persist for up to three weeks following the operation.
  • The eyes appear watery partly because of swelling under the conjunctiva and because the tear ducts are swollen and do not drain as readily.
  • You may find your eyes are temporarily sensitive to light; your surgeon may recommend eye drops to relieve temporary burning or itching sensations and you may want to wear dark sunglasses for a couple of weeks to protect your eyes from the wind and sun.
  • The incision lines will fade over several months until they become barely visible.
  • Occasionally a pool of blood can collect under the skin after the operation has finished (haematoma) this usually disperses spontaneously over 2 or 3 weeks but it may need to be drained if it is large. Quite commonly the margin of the lower lid is slightly pulled away from the eye during the first day or two after surgery due to swelling. This will settle on its own or with the help of suture strips or steri-strips. Very occasionally another operation is necessary.

Eyelid Surgery risks and possible complications

All surgery inevitably carries some uncertainty and risk, however they are small and infrequent.. Blepharoplasty is one of the most common cosmetic procedures performed each year, and it can be done very safely.

  • Hematoma : A hematoma is a collection of blood under the skin. Hematomas typically develop within the first few days after surgery. The body can absorb small hematomas. Larger ones require the placement of surgical drains for optimal healing.
  • Infection : Antibiotics will be given to you prophylactically before surgery and for several days after surgery to minimize the risk of infection.
  • Scarring : All new scars are red, dark pink, or purple. Scars of the face usually fade as they mature within 3 to 6 months. Our objective is to make them as inconspicuous as possible. In some cases broad or raised scars (keloids) develop. After the healing process is complete these can be revised.
  • Symmetry: Perfect symmetry is rare in nature. There may be some asymmetry between the eyelids.
  • Cysts : Sometimes tiny white cysts can appear along the stitch line. They are nothing to be concerned about but can be pricked out with a needle.
  • Eye Complications : Lid retraction is when the eyelid does not cover a sufficient amount of the eye. This condition may result in dry eyes. Saline tears will alleviate this situation. Lid retraction may also result in sclera show (this is when the whites of the eye are visible above and below the iris – colored portion of the eye). Ectropion is a rolling outward of the lower eyelid. This may result in excessive tearing, eye irritation, and will likely require corrective surgery. Blindness is an extremely rare complication.

The improvement in the appearance around the eyes will be immediately apparent following a blepharoplasty. However, there will be some bruising and swelling which may last several weeks. The scars will undergo a natural maturation process involving a series of colour and texture changes over a period usually varying from 6 months to 2 years and blepharoplasty scars usually heal exceedingly well. The final result can usually be judged at around 3-6 months following surgery. It is important to note that the normal age related effects on skin laxity continue to take place and over time will lead to a recurrence of the eyelid features affected by ageing.
